Short answer and full answer
What evidence does an assessor ask for?
The artefact that proves the practice was performed: a dated plan, a requirements baseline, an allocation matrix, a test record, an acceptance record, a change log or an evidence index with an owner for every row. The test is whether a named person can produce it on request.

The artefact that proves the practice was performed: a dated plan, a requirements baseline, an allocation matrix, a test record, an acceptance record, a change log or an evidence index with an owner for every row. The test is whether a named person can produce it on request.
The register names three artefacts per unit and the kit exports them as an index with owner, due date, location and status. The test is whether a named person can produce the artefact and explain the decision it records without asking someone else where it is. A policy that says the practice happens is not evidence that it happened. A file that lives in one person's mailbox is not a file.
Where the answer stops
This answer is written from the public ISO record and the register on this site. The FDIS body is not public, so the answer does not quote it or state a draft clause. It is not legal advice, does not certify and does not decide a question for a particular firm or system.
If the answer turns on a fact about your firm, use the free check or read the practice that names the evidence.
